I'm not here to give you an earful, I'm here to find the conditions you are keeping it in, figure out what's wrong and tell you how to correct it. I keep some super touchy centipedes that Ivories can't even compare to when it comes to care difficulty level like subcrustalis, Malaysian Jewel and a few others. You just have to mimic there natural environment and keep it clean. If you tell me what you are keeping it in, substrate depth, temp and humidity I can give you the corrections needed but it will be up to you to make them. I'd take the ivory off your hands but I'm dealing with over 200 centipedes at this point so I simply have no room.
We are here to help, even as many of us are still learning ourselves! You already realize that the conditions are not working so no one needs to tell you that. You might also take a look at @Cavedweller's caresheet as it should help you improve the conditions for your Ivory. Since four of the original five have already died, there is a good chance that this last one is on its way out as well but I still think you should try. Hopefully it can be saved, and then you can decide whether you want to keep millipedes after all or send it off in good health. As it stands, the millipede is stressed at the very least and shipping is an additional stress that it would probably not survive in its present condition.
