Sorry about the picture - I didn’t want to type it all out again. Also I’m so completely new to this website, just signed up, so I don’t know all the rules or how your supposed to do things. I’m just desperately trying to find somewhere I can get help with this.

just looking at these pictures makes me sick. I don’t want Sherman to be suffering. If anyone knows what I can do or what I’m doing wrong please let me know, thank you!

What is the temperature in the room where it is kept?

Search the board for banana slugs and you'll find some threads about them. I was just reading cool temps with high humidity are needed to avoid slowly withering away. Good luck to you and your slug.

it’s unfortunately usually around 74 F, I know how important it is to keep it cool and I put ice packs around the tank and leave all my fans on. Where I live is ridiculously hot all the time but it’s also super humid (I think I may have said it wasn’t in my original post, that was a mistake). Are there other ways to keep tanks cool? I don’t have any power over the temperature in my house.

Assuming it was already full grown when your class acquired it, there's no telling how old it is. A quick Google search says average lifespan for one of these is 1-7 years, so it is possible it's just reaching the end of it's life. I don't have any experience keeping these, so can't offer more help unfortunately.

try to find a better way to insulate the enclosure and monitor temperatures. I believe others have said they are very temperature sensitive.
try to move the enclosure to a basement or insulated container to keep it cool. an old cooler or tub filled with insulating material might help. there are some people on caudata.org that keep heat-sensitive salamanders in complex setups successfully, even in warm climates.

It is also very possible that it was already at the end of its life when you acquired it, and that you did a great job with it for the past year.
