There is no form.
Pretty much all ornatas will have some purple on them.
Not really. There are ornatas that have a TON more purple on them than others. Some have far more green and almost nothing when it comes to purple.

That is true I have one ornata that showed lots of purple when young.
Now she is just dark green with alittle purple along the legs. And I have never read about a purple form.

What striping are you talking about?
All ornatas have yellow and white striping on their ventral side.
There is a guy(BEN-V) that sent me a Diagram of the Ventral striping on a normal Ornata and a Light Form, there is a difference in striping on Leg IV.I'm just going by what I've seen and heard, could prove to be nothing but as for now that is what I will call her.

I've been doing more reading on it now and there is a lot of fighting about Taxonomy of the "Light Form",it's still up in the air.Do a Google Search on it and you will see what I mean on the T-store.com site.

Ya I don't know that there is an "official" difference but there certainly a visible difference. I have read all color "forms" if you will have come from the same sac.
