It may be dehydrated if it’s only getting water every 1-2 weeks. Have you tried putting it’s fangs in a water dish yet?

Is that sprayed water every two weeks the only source of water it gets? If so, dehydration is the likely cause; they need access to fresh water DAILY....but there might be other factors in play.
Can you post pics of it & the enclosure it's in?

Blurry, but looks fat to me, perhaps it's flipping over to molt, and getting interrupted...

I don't see anything wrong with the set-up, looks like you've got ventilation, it's a decent size, etc. As mentioned, it might be getting ready to molt, &/or it might be thirsty. It should have access to water every day, while it might go weeks without food even if it's offered regularly. Make sure she's got water first & foremost, & see how she manages from there.

Looks like it’s about to molt, I would leave it alone

Is that real cedar in there? If so, I would for sure start by removing it. I think cedar and other pine woods/sprigs are toxic to tarantulas or are at the very least a deterrent that they don't like to be around.

This is one place I saw that, but it's an old thread: https://arachnoboards.com/threads/are-tarantulas-alergic-to-cedar.127765/

If you search "cedar" in the forums, lots more will come up.
