How many slings does a Kochiana brunnipes have? Sorry if this is common knowledge on the forum here, but I've been googling this and cant find an answer.

Are the legs curled under its body? That sounds like a death curl, how much ventilation does it have? All Avicularias and ex Avicularias need a lot of ventilation but there still old care sheets out there that say they need a lot of humidity, which actually ends up killing them.
Was it in a waterdish? Maybe thats why the opisthosoma is wet. Sometimes the T will chew the molt up so maybe it was about to do that but decided not to and thats how the holes got there. Not really sure what the black spots could be.
