Everything sounds good, I would raise the temps to 90 ish on the hot side. No extra lighting is nessesary. Make sure she gets vitamins as well as calcium, less often though. If u need more detailed info just ask.
I just put mine in warm water and they flatten out, it's pretty cool. Some people add soap I think. U can use pins to move the legs into the position u want. It's better to do this when the most is fresh btw.

Three kicks can still create a bald patch. They don't hurt the tarantula at all. If she is falling you can add more substrate so the height from the substrate to the top of the enclosure is no more than 1.5 times her leg span.
Very pretty! My O. ornatas stays above ground and never burrows even thought she has substrate 2 times as deep as she is long. I keep her on moist substrate and she gets along well.
