Sounds like you were giving him good care.
Six inches qualifies as an adult size, although some forms are larger.
If you bought him at a pet store he was
almost certainly wild-caught, and a WC adult could be any age.
Your best bet is that he died of simple old age. ;)

Thanks Thaedion :) As to the wood chunks, this is very light rotted wood from a firewood pile, broken into one inch and half inch chunks. With very little misting, these chunks are saturated to the point that water flows freely from them when squeezed by hand. Nevertheless, I'm here to seek...
