I'm thinking Avicularia avicularia m#6.
If female, the spermatheca of juruensis m#2 (ex. urticans) is slightly different than Avicularia avicularia but that'll take a decent macro lens or microscope and some good molt manipulation. The easier way to tell is avicularia leg IV (hind) is %10 longer than leg I (front), whereas juruensis the two sets of legs are the same length.
@Drea , thank you, but I'll defer to @CEC as he has supplied some very good taxonomic differences to help everybody process these different morphotypes of the Avicularia avicularia species that is causing a lot of headaches. @CEC has posted some awesome pics to help everybody see these differences as well.
